St. Francis de Sales School is a Catholic school located in the heart of Southwest Philadelphia. We serve students in over 30 zip codes in Philadelphia and Delaware Counties. We accept students regardless of their race, creed or national origin. We serve 450 children, in grades Prekindergarten through eighth grade.

Registration forms can be found below on this web page. Additional documents are required to complete registration. Registration paperwork must be submitted as a complete packet, including all of the documents listed below.

• Birth Certificate
  • Pre-Kindergarten – must be 4 before September 1
  • Kindergarten - must be 5 before September 1
  • First Grade - must be 6 before September 1 and have completed a kindergarten program (keeping in mind the age requirement).
• Up-to-date Shot Records and an updated Physical Exam.
  • If your child or children are not due for a Physical at the time of registration, please bring the most recent one they had.
• If your child has to receive medicine in school then please download the administer medicine form and have it completed by your child’s physician. 
• Proof of Address (eg. Driver’s License, utility bill)
• Baptismal Certificate (if applicable)
• Latest Report Card for Grades 1-8
• Terra Nova Test Scores/PSSA Scores from Public Schools or your states’ standardized scores.
• $150 registration fee ($25 for any more children being registered after the first)
  • Can be paid with cash or money order

Testing
Students starting in Kindergarten to eighth grade will be tested on the day specified below. This will be scheduled at your registration appointment. (Pre-K will not be tested.)

• Grades K – 3 – Will take place on a Wednesday
• Grades 4 – 8 – Will take place on a Thursday

Tests will consist of three components: Mathematics, Reading, and Writing. These tests will indicate if your child is on grade level and prepared for the upcoming school year. In addition to our testing, report cards, grades and behavior from the previous school will be carefully reviewed. All students are expected to have a satisfactory behavior report. Testing appointments will be scheduled once registration is complete.
 

FACTS Registration
Upon acceptance to St. Francis de Sales School, you will set up a FACTS TUITION ACCOUNT. The Tuition Account must be created within 10 days of your child’s acceptance. Tuition is paid via FACTS Tuition Management System. Families can decide whether or not to pay tuition in one block or over 10 months.  Please see the tuition documents below for more information.
